Heart valve surgery in the United Kingdom: present practice and future trends.

چکیده
Editorial Heart valve surgery in the United Kingdom: present practice and future trends The surgery of heart valve disease raises many questions. Should we conserve or replace the diseased valve? What should the replacement valve be-a homograft, a biopros-thetic valve, or a mechanical valve? Isn't valve disease becoming less common anyway? Test reports, clinical comparisons, and longitudinal follow up studies, particularly those focusing on the very latest artificial heart valve are reminiscent of car tests in motoring magazines-with the latest model, incorporating state of the art technology, giving superior performance (under manufacturer's test conditions) ... and costing just that bit more. This plethora ofscientific advice about which valve to choose can be confusing and individual reports are not necessarily widely representative or capable of extrapolation. Comprehensive information on the pattern ofoperations for heart valve replacement has been available on a national basis in the United Kingdom since 1986, when the United Kingdom Heart Valve Registry was established. This registry, funded by the Department of Health, receives data from all National Health Service cardiac surgical units in the United Kingdom and provides unique information on the changes in patient numbers, on demographics data, and on the preferred types of artificial valves currently being implanted by cardiac surgeons in the United Kingdom. Although the national registry cannot yet be expected to answer the vexed question, "what artificial heart valve should we use?", it already provides answers to other questions.

The objective of this report is to present a young patient with combined mitral valve and coronary artery disease to illustrate these challenges. In this report, we present a 47-year-old man with longstanding HIV infection who was referred with severe mitral regurgitation (MR) and profound...
متن کاملThe Role of MRI in the Diagnosis of Post-sternotomy Injuries of the Brachial Plexus
Brachial Plexus Injury (BPI) is an uncommon complication of median sternotomy capable of causing a permanent or transitory sensitivity and/or motor function impairment in the upper limbs. During a cardiac surgery through sternotomy, for the assessment of the thoracic cage configuration and the site of mediastinal structures, a broader surgical field may be required. If the sternal retractors ar...

This case report was an attempt to present the importance of diagnostic echocardiography and closure of a patent foramen ovale in carcinoid heart disease. Herein, we present a case of a 48-year-old woman with a carcinoid metastasis presenting with tricuspid regurgitation, pulmonic stenosis, patent foramen ovale, and borderline left ventricular ejection fraction on an echocardiogram. Surgical in...
متن کاملHealth-Related Quality of Life after Valve Replacement Surgery
Background: Heart valve disease gradually increases the pressure in the heart chambers and impairs heart pumping. It influences on quality of life due to low output, disability, fear, amnesia, anxiety, and depression. Although most of signs and symptoms in patients with heart valve diseases may significantly decrease after valve replacement surgery, there is limited information about the impact...
متن کاملRight ventricular Hemodynamic Alteration after Pulmonary Valve Replacement in Children with Congenital Heart Disease
Introduction: In patients who underwent surgery to repair Tetralogy of Fallot, right ventricular dilation from pulmonary regurgitation may be result in right ventricular failure, arrhythmias and cardiac arrest. Hence, pulmonary valve replacement may be necessary to reduce right ventricular volume overload. The aim of present study was to assess the effects of pulmonary valve replacement on rig...
